Accuracy of Information
I certify that all business materials, promotional materials, product images, service descriptions, logos, links, and submitted assets submitted in this form are true, accurate, current, and authorized for use. I understand that false, incomplete, misleading, or unauthorized submissions may result in removal from the Official Small Business Partner program without refund.
Payment and No Guaranteed Outcome
I understand that the selected Official Small Business Partner investment is a promotional partnership payment and does not guarantee sales, customers, vendor approval, exclusive selling rights, sponsorship opportunities, referrals, followers, website traffic, event attendance, press coverage, business opportunities, or any specific financial or promotional result.
Refund and Cancellation Policy
I understand that all Official Small Business Partner payments are final and non-refundable once submitted, except where Avidity cancels the program entirely and chooses, in its sole discretion or as required by law, to issue a refund or credit.
Review and Approval
I understand that Avidity reserves the right to review, approve, deny, edit, omit, reschedule, substitute, or modify any submitted business materials, product or service descriptions, logos, images, listings, partner-related content, display opportunities, or partnership benefits for operational, legal, editorial, technical, timing, reputational, space limitation, or event-related reasons.
Music Rights and License
I represent and warrant that I own or control all rights, permissions, licenses, and clearances necessary for Avidity Gospel Music Awards, Avidity Recording Academy, Avidity Promotions, Avidity Media, LLC, their representatives, contractors, media partners, successors, and assigns to play, stream, display, post, reproduce, distribute, edit, identify, promote, and otherwise use the submitted song, artist name, image, likeness, logo, biography, website link, and social media handles in connection with the Official Small Business Partner program, the Pre-Telecast Show, event promotion, website listings, social media, digital media, recap content, advertising, and archival materials without additional compensation.
Promotional Release
I grant Avidity Gospel Music Awards, Avidity Recording Academy, Avidity Promotions, Avidity Media, LLC, and their representatives permission to use my name, stage name, organization name, image, likeness, voice, biography, logo, submitted materials, event photographs, event video, interviews, testimonials, and related content for promotional, advertising, archival, editorial, social media, website, broadcast, streaming, and public relations purposes without additional payment, approval, or notice, unless prohibited by law.
Brand and Badge Usage
I understand that any Official Small Business Partner badge, Avidity name, logo, event name, artwork, or brand asset provided to me remains the property of its respective owner. I may use approved Small Business Partner badge materials only to truthfully promote my participation in the Official Small Business Partner program and may not alter, sell, transfer, sublicense, imply endorsement beyond the partnership, or use Avidity branding in a misleading, offensive, unlawful, or unauthorized manner.
Code of Conduct
I agree to conduct myself professionally and respectfully in connection with the Official Small Business Partner program, Avidity events, online promotion, event venues, staff, volunteers, attendees, nominees, performers, partners, sponsors, and media representatives. Avidity may deny access, revoke benefits, remove listings, or end participation without refund for harassment, threats, discrimination, unsafe behavior, reputational harm, policy violations, disruptive conduct, or unlawful activity.
Limitation of Liability and Release
To the fullest extent permitted by law, I release and agree not to hold Avidity Gospel Music Awards, Avidity Recording Academy, Avidity Promotions, Avidity Media, LLC, their owners, officers, directors, employees, contractors, volunteers, representatives, partners, sponsors, vendors, venues, affiliates, successors, and assigns liable for indirect, incidental, special, consequential, punitive, or lost-profit damages, or for technical issues, scheduling changes, force majeure events, platform outages, third-party acts, travel costs, lost opportunities, or unmet expectations related to the Official Small Business Partner program.
Indemnification
I agree to defend, indemnify, and hold harmless Avidity Gospel Music Awards, Avidity Recording Academy, Avidity Promotions, Avidity Media, LLC, and their representatives from and against any claims, losses, liabilities, damages, costs, and expenses, including reasonable attorneys’ fees, arising from my submitted business materials, product/service claims, submitted assets, intellectual property issues, sales-related claims, or conduct connected to participation
Business Materials Rights and License

I grant Avidity Gospel Music Awards, Avidity Recording Academy, Avidity Promotions, Avidity Media, LLC, and their representatives permission to display, post, reproduce, distribute, edit, identify, promote, and otherwise use the submitted business name, brand name, image, likeness, logo, biography, website link, product or service descriptions, social media handles, and submitted business materials in connection with the Official Small Business Partner program, event promotion, website listings, social media, digital media, recap content, advertising, and archival materials without additional compensation.
Privacy and Data Use
I understand that the information I submit may be collected, stored, processed, and shared as needed to administer the Official Small Business Partner program, process payments, provide tickets, verify submissions, communicate with me, publish approved listings, promote the event, maintain records, and comply with legal or operational obligations. Payment details are processed by the selected third-party payment processor and should not be collected manually in this form.
